Such a box is called „panel“ and it’s the knife the evil doctor uses. By the way I spend too much time in front the screen- I have problems with reading anything. Geez looks like I need glasses NOOOOOOOOOOOO!!

Look at the area the blood spurts. If the knife (or whatever it is) is going into the carrot, then the blood should be on the part of the knife leading into the carrot. If it's just a slash, then it would be splattered somewhat around the knife, carrot and floor. Besides, the way the knife is embedded, it certainly don't look like a slash. Now if it was a stab, the blood would be on the tip of the knife, leading back, and a bit deeper into the knife if it was pulled out.
Thus, I conclude that the blood isn't the carrot's or that the carrot was brutally struck with one strong jab.
